The Department of Health Care Finance (DHCF) is providing a two-day workshop for My Health GPS providers. This workshop will offer an interactive training experience, including an overview of key program requirements, networking with My Health GPS care teams and community organizations; and customized technical assistance on topics such as workflow redesign and care planning.

Purpose
The purpose of this workshop is to highlight essential program information, share best practices, and provide customized support to providers for the success and sustainability of the My Health GPS program at their facility.

Who Should Attend
This workshop is designed for all members of the My Health GPS team. Prior to the workshop, a detailed agenda will be provided which will indicate which sessions may be of most interest to particular groups of attendees (e.g. care coordinators; health home directors)

Meet and connect with the My Health GPS community
Gain a full understanding of the My Health GPS program requirements
Receive “in the moment” technical assistance on best practices for operations, care coordination and population health management
