Attendees will learn strategies to drive evidence-based medication utilization and support patient access to medications through the implementation and evaluation of quality improvement projects.
After this activity, the following patient outcomes are expected to be improved: medication access and utilization, patient experience, treatment adherence, care transitions, post-discharge support, patient education, reduced readmissions, and health equity.
