MOLST: Meeting the Challenge

Date: Oct. 18, 2017

Time: 8:30am- to 11:00am. Arrival Time: 8am for Continental breakfast and registration.

Where: The First Unitarian Church of Worcester, 90 Main Street, Worcester

Welcome EMS personnel! 2.5 CE Credits for EMS Personnel available.

MOLST, Medical Orders for Life-Sustaining Treatment, is a powerful tool for frail and seriously ill adults to make informed end of life care choices, record their choices, and be assured their choices will be honored at home, on transfer, and across all care settings.

Our challenge is to build a collaborative MOLST process that is responsive to an adult’s changing health needs and that honors care choices through their health journey and at the end of life.

Come join us for a unique forum bringing together state leaders & care professionals.

The panel will discuss best practices and address implementation challenges to consistently know & honor consumer choices.

Engage in Round Table Discussions: Let's Build a MOLST Process Together!

Following the panel, share your expertise with a diverse cross-section of care & community professionals and suggest ways to improve the MOLST form; work with colleagues to ‘build your team’ to engage adults & families in discussions; and align protocols & policy measures to consistently provide quality person-centered care. Together we’ll generate guiding principles and next steps to craft a responsive process for all MA adults.

What You'll Learn:

- Fact vs Myth: MA law & state regulations

- The MOLST Form: Understanding choices, making it valid!

- Build a Team: The role & decision-making authority of physicians, care professionals, Health Care Agents, Guardians

- Tools for effective, culturally sensitive planning discussions

- Strategies to align our protocols to know and honor consumer choices, across all settings

You Should Attend:

Physicians, Nurses, Social Workers, EMS, Community Groups, Clergy, Assisted Living, Skilled Nursing & Hospice Staff - ALL care providers engaging in health care discussions, providing care, and taking part in the MOLST process.

No Cost. Must register to attend.

For EMS only: Workshop is free; if seeking 2.5 Credit, the cost is $31 paid to Bershire AHEC. You can register andpay on-line today. After the wrokshop is complet they will send you your certification. Read details below.

EMS CE CREDIT INFORMATION. This program has been approved for 2.5 Continuing Education Program Hours by the Massachusetts Department of Public Health, Office of Emergency Medical Services. Successful Completion Requirements: Participants are expected to sign out at the conference, attend the entire program, and complete and submit the conference evaluation to receive a certificate. Certificates will be mailed to the address provided within 4-6 weeks of the conference date.
