The Regional Family Support Planning Council #6 (Monmouth, Ocean) cordially invites you to a community session designed to equip families with practical tools to care for their loved ones with disabilities.

Christine Bakter, Deputy Director of the New Jersey Office of the Ombudsman for Individuals with Intellectual or Developmental Disabilities and Their Families, will share updates and advocacy insights from across New Jersey’s systems of care.

Representatives from Northwell Health will introduce the Map Our Life study. This research study aims to help families build personalized long-term plans for their loved ones with Intellectual or Developmental Disabilities (I/DD) and improve well-being. Eligible participants will have the opportunity to explore a web-based interactive tool that covers topics such as emergency planning, caregiver support, and care coordination.

This IN-PERSON event is free and open to individuals with I/DD and their families seeking actionable strategies and a deeper connection in the community.
