Introduction to Advance Care Planning - Community Class

This class provides an opportunity to learn about advance care planning and advance directives. You'll get tips on how to select a health care surrogate and complete a living will. We'll also discuss how to ensure that your care providers know what is important to you about your health care.

We will use a virtual format called Microsoft Teams to present the class. A link to the Microsoft Teams invitation will be provided after your register for a class session.

There are several ways you can connect and participate. You can join using your computer, laptop, tablet, smartphone, or any phone by simply calling in. If you would like to see our speaker, please join with a computer or smartphone. We would love to see you too! If you feel comfortable, we invite you to enable your video when joining.
