Public Partnerships invites Individuals, and their Authorized Representative if applicable, already enrolled in DDD’s Self-Directed Option to join us at one of our in-person enrollment sessions available in June. If you are unable to enroll with us online or over-the-phone, our staff will be glad to enroll you at one of these in-person enrollment sessions.

Instructions to Enrollment Session Attendees

To ensure the successful completion of your enrollment process, please note the following points:

1. If the Individual is also enrolled in the Personal Preference Program (PPP), please note that the Individual CANNOT enroll as the Employer of Record. Another person must attend and serve as the Employer of Record.

2. If the Individual is only receiving services from an Agency and does not currently have a Self-Directed Employee through ESNJ, the Individual does not need to attend this enrollment session. The Individual will be automatically enrolled and no further action will be required.

3. Public Partnerships requires an e-mail to enroll you. Please provide a valid e-mail at the enrollment session.

4. If you intend to sign enrollment forms on behalf of the Individual as a Legal Guardian or Power of Attorney, please bring all supporting paperwork to the enrollment session.

5. The Individual’s Self-Directed Employee cannot enroll at the enrollment session without the attendance of the Individual and/or Employer of Record.

6. The Individual's parent, guardian or spouse CANNOT be a Self-Directed Employee in the NJ DDD Self-Directed Option.

7. Self-Directed Employees who provide transportation services must provide a Driver's License at the enrollment session.
