Overview
Join Vital Signs on Wednesday, December 10, 2025, for an opportunity to collaborate with community organizations committed to taking action on three key health priorities that impact our communities: Behavioral Health, Injury & Violence Prevention, and Chronic Disease.
This meeting will focus on Chronic Disease, fostering meaningful dialogue to better understand existing efforts, identify available resources, and determine where alignment can create the greatest collective impact in advancing strategies for improving community health.
What to Expect at Our Next Meeting
At this meeting, we will:
• Identify how partner activities align with Chronic Disease Strategies 2 and 3
• Explore resource capacity and collaboration opportunities
• Select a focus strategy and begin drafting a SMART objective to guide collective action
If you are planning to attend this meeting, you must complete the Partner Resource Questionnaire by December 1. The information you provide will help identify existing resources, align efforts, and inform how we move forward together to address the Chronic Disease strategies.
