HCN Peer Collaborative for 2026
Overview
We are excited to continue the focus on and support of our SSVF Health Care Navigators (HCN) and the vital role they play with our Veteran households to:
- Facilitate access to care,
- Reduce barriers,
- Coordinate care,
- Deliver health literacy support, and
- Advocate
Beginning in January 2026, we will be launching a nationwide Health Care Navigator Peer Collaborative which will include monthly calls facilitated by HCN peers and TA teams. The target audience for these sessions are SSVF Health Care Navigators. The meetings will occur at a regular cadence, and all grantees have been divided across two cohorts (East and West). We are offering two kick-off calls in January to provide an overview, the goals, an outline of the sessions and support available within the HCN Peer Collaboratives. We encourage all grantees to register for one of the kick-off call options, whichever one works best for your schedule! Please note that ongoing calls will be based on pre-designated assignments of grantees to their cohort.
When you enter registration information, please include the contact information for your agency’s Health Care Navigator lead point of contact (POC)
· This does not have to necessarily be your team’s HCN, it simply needs to be the person that will be responsible for receiving the meeting links and ensuring they are distributed to the Health Care Navigator(s) on your SSVF team.
· REMINDER: The target audience for these sessions are SSVF Health Care Navigators, please do not forward along to your entire team and if necessary, share with discretion.

Your access code is your grant ID with no dashes.
For example, if your grant ID is 12-GA-119, your access code will be 12GA119.
