The film follows Duane Bradley, a seemingly mild-mannered young man who arrives in New York City and takes up residence in a cheap, rundown hotel in Times Square. With him, he carries a large, locked wicker basket. Inside the basket is Belial, Duane's hideously deformed, telepathic, and monstrously violent separated Siamese twin. The brothers share a dangerous bond and a singular purpose: to track down and gruesomely murder the surgeons who performed the unwanted separation when they were children. As Duane tries to pursue a relationship with a local woman, Belial's jealousy and need for bloodshed escalate, leading to a series of bizarre and horrific confrontations.
