We FUTURE DOCTORS recognize that healthcare in Hawaii is in desperate need of reform. Some of the interlinked problems of Hawaii’s healthcare include the following:

  • Rapidly increasing healthcare administrative burdens and costs

  • “Quality”measured and rewarded via “Pay for Performance (P4P),” which relies on data and complex computerized algorithms controlled by insurance companies and government agencies

  • Electronic Health Records (EHR) filled with data irrelevant to patient care, without the interoperability to make it truly useful

  • Prior authorizations, much of which is performed by insurance corporation employees with little to no understanding of the medicine involved. They override physicians’ medical judgment, waste doctors’ valuable time, and block patients from obtaining needed care

  • Too many patients to see/not enough doctors

  • Too little reimbursement to support care teams

  • Doctors’ incentives become aligned with those of insurance corporations → loss of intrinsic motivation to serve → physician burnout

  • Worsening doctor shortage

  • Older doctors retiring early

  • Younger doctors not entering primary care

  • Decreasing number of doctors taking in Medicare patients while patient population with Medicare is growing

  • High cost of medications, restricted formularies

  • High insurance premiums, high out-of-pocket payments

  • Unnecessary and expensive ER utilization

We will most likely inherit these problems if no fundamental changes in policies are implemented. How we respond to these healthcare problems will determine the future of health of the people of Hawaii.

However, we believe we can find ways to effectively address Hawaii’s healthcare problems while taking into account of our State’s unique healthcare situation. And we believe we can achieve healthcare for everyone - if we get it right.

WE WANT TO HEAR FROM YOU! Also, hear what our health care policy experts have to say and voice your opinions and ideas with them. Let’s talk about solutions, how we can work together to make changes happen. We believe that our proposals can become ACTION and that Hawaii doctors can powerfully influence our State politics and the future of healthcare in this country.
