The Critical Role of Follow Up and Care Coordination in Suicide Safer Pathways

The period between discharge from an inpatient mental health stay to connecting with an outpatient provider is an incredibly high risk time for one to die from a suicide death. This webinar will explain filling the gaps in care during this time can lead to positive outcomes in this population. The best strategies for both inpatient and outpatient mental health providers will be outlined so that there is a shared understanding that it takes each working together to provide this most sound post-stay care.

Examples of how this is done in an inpatient and outpatient setting will be reviewed. Resources including tools to assist in readiness and action planning will be provided.
