OVERVIEW

Niagara County joined the NYS Success Learning Collaborative in the second year (fall 2014) of the NYS Success grant. The SOC team has engaged multiple stakeholders, as well as youth and families, who have joined the cross-system workgroup. Niagara County participated in the NYS Success System of Care Readiness Assessment and Implementation Measurement Scale (SOC-RIMS) pilot to “determine the readiness status, so that planning and implementation can be based on the identified strengths and weaknesses of the current community system, as perceived by stakeholders”. Then the Niagara SOC team used its follow up SOC-RIMS findings from February 2016 to guide its continued SOC work.

Mission: It is the mission of the CNOC in Niagara County to identify current services for children and their families, as well as the gaps in services, and encourage collaboration among all.

Vision: It is the vision of the CNOC in Niagara County to provide a coordinated network of services and supports that meets the needs of children and families in Niagara County. These services and supports are child and family directed; community-based; and respectful to situations, cultures, privacy, and primary language spoken.

GOALS

  • Build a cross-system System of Care
  • Develop guiding principles that incorporate the SOC values and principles
  • Increase Cultural & Linguistic Competence of child and family serving providers within the community
  • Incorporate SOC work into the Niagara County Plan
