Description

This is the second part of a three-part series, this second training focuses on the importance of documentation. How to write case notes and how to use the DAP model in your case management, why it's important to document, and why consulting is essential.

Learning Objectives

Participants will be able to:

  • Identify the importance of consulting with colleagues
  • Demonstrate the knowledge of the importance of documentation.
  • Apply the use of the DAP model in their case management documentation.

Presenter

Imelda Gonzalez - Hola mi Gente! My name is Imelda and I was born and raised in San Francisco’s Mission & Excelsior District and recently moved to San Joaquin County. I am a mother, leader, warrior, advocate and I am BLESSED. As a daughter of undocumented parents, I have overcome many obstacles that an English language learner struggles with. I was a case manager before having the title, as a child, I had to help my family with translation, health care, social services and learn how to navigate the system. I obtained my AA in Liberal Arts at City College of San Francisco. I am currently enrolled in Stanislaus State College pursuing a major in Criminal Justice. I have 17 total years of experience working closely with justice-involved youth, in the community, SFUSD settings, and within the SF & Stockton Juvenile Justice System. I myself was involved in case management services at the age of 14years old up until a young adult. Having a strong and caring case manager as an ally can help mold and change your perspective.
