Advance Care Planning Training Announcement 2022

The Foundation for Healthy Communities is offering a four-part training series on facilitating Advance Care Planning (ACP) for health care and related professionals.

It is suitable for professionals familiar with and new to Advance Care Planning and will address:

• Basic concepts of ACP and history of Advance Directives

• Detailed attention to the Advance Directive forms and rules pertaining

• Skills in introducing and informing consumers about Advance Care Planning

• Skills in advising health care colleagues about Advance Directives, Medical surrogacy, and guardianship

• Recent changes in Legislation

Participants will be expected to attend all four live sessions. Sessions will be interactive; participation is required. Zoom links will be provided upon registration. Pre-session assignments will made, and materials provided between sessions.

Dates: November 28, December 5, 12 and 19, 2022 12:00 – 1:30 p.m.
