Description

The Capital Area Health Alliance (CAHA) is pleased to announce its second education and networking event related to whole person care. This session will focus on Social Determinants of Health.

The program will open with an overview of the Healthy! Capital Counties 2018 Community Needs Assessment, focusing on findings related to Social Determinants of Health. Two organizations – the Ingham County Community Health Centers and the Northwest Michigan Health Initiative - will share their perspective and experience with screening for social determinants. They will also discuss how they are using the information to improve population health and address the complex needs of their patients.

Learning Outcome:

1) Participants will recognize the social, economic and environmental factors that influence community health needs.

2) Participants will gain increased knowledge of how information about social determinants of health is being collected and utilized to improve health outcomes and population health.
