Description

Hear about initiatives that guide patients and families in end-of- life decisions and identify how each member of the care team may contribute to the beginning conversation. This lecture will also discuss our role in the Conversation project and how we can be become a conversation ready hospital; methods that may help healthcare providers begin conversations with patient and families about health care decisions during serious illness and end-of- life care.
