Westview Class of 2023 Grad Party

Westview Class of 2023 Grad Party Waiver:  Student and Parent please read.

The student named in this online form/document has my permission to attend the 2023 Westview High School Grad Party June 10, 2023, sponsored by the 2023 Westview High School Parent Teacher Organization (PTO).  By my signature below, I acknowledge that the Beaverton School District is not a sponsor of or affiliated with this event.  I agree to indemnify and hold harmless the Beaverton School District, it agents, and employees, the organizers of this event and the officers of the Westview High School PTO, from any loss, damage, injury, claim or demand arising from this activity related to or in conjunction with the 2023 Westview High School Grad Party.

I understand and acknowledge that:

1. The term “hosting facility,” as defined in the waiver, is the location where the grad night event will take place.

2. Any use of the facility other than the designated areas is strictly prohibited; areas will be clearly marked and communicated. Loud, offensive, abusive, profane, or bothersome behavior to fellow participants, chaperones and/or staff will not be tolerated. This includes fighting.  If participants choose to leave the event early, they will not be re-admitted.  Non-participants will not be admitted.

3. The event is for those students eligible to participate in the 2023 Westview High School Graduation ceremony on June 10, 2023, and who have turned in all required documents to the 2023 Grad Party committee.  Non-participants will not be admitted.  Students are required to ride the bus to and from the event and will not be allowed to leave the event early unless there are prior arrangements and approval from the event coordinators.

4. This is a drug-free and alcohol-free event.  Students will be required to pass a breathalyzer test and undergo a check of their possessions to ensure they are not bringing prohibited substances or material on the bus or into the hosting facility.  Students found to be under the influence, caught using, or in possession of controlled substances at any point during the event will be immediately removed and his/her parents will be notified

Student Acknowledgement - By typing my name in the Student Acknowledgement section at checkout, it will act as my signature acknowledging that I agree to adhere to the Code of Conduct in the Waiver (above) and understand that any violation of the expectation set forth by the facility and 2023 Westview High School Grad Night committee will result in my expulsion from the event.

Parent Acknowledgement - By typing my name in the Parent Acknowledgement section at checkout, it will act as my signature acknowledging that I agree to the Code of Conduct Waiver (above) set forth by the facility and 2023 Westview High School Grad Night committee and agree to pick up my student should he/she need to be removed from the event at any time.
