Health care has experienced a number of key shifts since the year 2000. Driven by patients and others dissatisfied with the status quo, commercial pressures coming from corporate responses to inflation, rapidly changing technology, and more, palliative care is seeing these shifts as well.

To address some of these major health care trends, Dr. Wells Shoemaker will walk us through 8 of the most impactful changes and how they specifically relate to and affect palliative care. Judy Thomas will join Dr. Shoemaker to provide a year-end retrospective of health care in 2017.

1 CEU included for nurses and social workers.


 Webinar participants will be able to:

  • Describe "person-centered care" and its current impact on healthcare delivery

  • Discuss the validity of quality measures and the role they play in patient well-being

  • Describe how the role of the physician has changed since 2000

Judy Thomas, JD, is CEO of the Coalition for Compassionate Care of California, a partnership of healthcare providers, non-profit organizations and associations, government agencies and individuals working to ensure seriously ill people receive quality, compassionate care. Thomas chairs the National POLST Paradigm Task Force and serves on the Donate Life California advisory board. In 2015, Thomas was named a Practice Change Leader by the John A. Hartford Foundation.


F. Wells Shoemaker, MD
Independent Healthcare Consultant

Wells Shoemaker practiced pediatrics on the Central Coast for 25 years, devoting the subsequent 20 to quality improvement on an expanding statewide scale. He served as CAPG’s medical director for 8 years and co-chair of the California Quality Collaborative for 9 years.  He participated in Governor Brown’s Let’s Get Healthy California (LGHC) taskforce in 2012, and concurrently launched the California Advanced Primary Care Institute.  Dr. Shoemaker then served in 2013 as co-lead of the follow up LGHC workgroup charged with ‘Redesigning the health system for safe, effective, patient centered care.’ He has been a member of the CCCC Board of Directors since 2012, currently serving as Secretary.


CONTINUING EDUCATION

Full attendance at the live webinar (you must attend a minimum of 50 minutes) is a prerequisite for receiving professional continuing education credit. Attendees requesting CEU credit must each call into the webinar individually using the call-in number and computer link from your instruction email for your attendance to be documented. If you do not follow these directions, there is no official documentation of your attendance, and you will not receive CEU credit for this webinar. Ask us about special arrangements offered to CCCC organization members.

You are also required to complete an online survey within 30 days following the webinar to receive a a CEU certificate or certificate of attendance. A link to the evaluation will be provided in an email sent to you within 24 hours following the webinar. The email will also include links to the presentation recording and slides. Certificates will be sent via email.

Nursing: Provider approved by the California Board of Registered Nursing, Provider #CEP 15403, for one (1) contact hour.
Social Work: Course meets the qualifications for one (1) hour of continuing education credit for LMFTs, LCSWs, LPCCs and/or LEPs as required by the California Board of Behavioral Sciences. CCCC is approved by the California Association of Marriage and Family Therapists to sponsor continuing education for LCSWs and LMFTs, Provider #91987.
Chaplains – This program may be used for continuing education credit for chaplains certified with the Board of Chaplaincy Certification Inc.

No refunds will be made after this date. Substitutions are encouraged and can be made by calling (916) 489-2222.
