Description
Our community will be introducing the Windsor Essex By-Names Prioritized List (BNPL) in the coming months which will include the use of the Vulnerability Index – Service Prioritization Decision Assistance Tool (VI-SPDAT). This common triage tool will help our community by:
· Knowing anyone experiencing homelessness by name.
· Understanding the history and needs of individuals experiencing homelessness which will aid in referring them to housing services and other supports as quickly as possible.
· Will create a systemic way of consistently updating and tracking individuals experiencing homelessness in our community.
· Will improve planning and optimize the allocation of resources based on the information collected.
In order for someone to be added to the By-Names Prioritized List, eligibility must be determined by:
· Completing the By-Names Prioritized List Addition and Consent Form; and VI-SPDAT (Vulnerability Index – Service Prioritization Decision Assistance Tool);
· Desire to participate is demonstrated by the individual;
· Individual is Imminently Houseable (documentation has been completed for those matched to a specific program)
The frontline staff who will be responsible for completing the By-Names Prioritization List Addition/Update and Consent Formand/or the VI-SPDAT must attend mandatory training regardless of familiarity with either form. The training will be separated into two parts which will start with VI-SPDAT.
While we know that many agencies are already using the VI-SPDAT, it is a requirement that everyone within your organization who will be conducting the VI-SPDAT and/or placing people onto the By-Names Prioritization List be trained/retrained. This will ensure that this tool is being used consistently and correctly within all agencies.
