We are celebrating the 6th annual VET WEEK initiative.

About this Event

The City of Riverdale Cultural Affairs Department designed a Veterans Initiative for the local community soldiers. Our mission is to create a social and recreation environment amongst comrades where the Veterans could come together and share stories, contact information and connect with beneficial support organizations that could provide assistance with veteran benefits.

Photo/Video Release Form and Waiver Agreement

To the best of my knowledge, I am acknowledging, that I understand, The City of Riverdale, {The Riverdale Town Centre, Town Center} can use and has permission to capture my image and likeness in photographs, videotapes, motion pictures, recordings, or any other media outlets (collectively “Images”), while visiting or participating in any event at the facility/grounds/complex/virtual platforms.

If I am on property, I am fully aware of the risks and hazards, including all possibilities, connected with the current CDC guidelines based on what is currently known as the Coronavirus Disease 2019 (COVID-19) suggest interim guidelines for facilities of event space that accommodate mass gatherings and large community events using your facility/grounds/complex, including physical injury or even death, if I herby elect to voluntarily participate in using your facility/grounds/complex, knowing that the associated physical activity may be hazardous to me and my property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OR LOSS, PROPERTY DAMAGE, OR PERSONAL INJURY, INCLUDING DEATH, that may be sustained by me, or loss or damage to property owned by me, as a result of utilizing Riverdale Centre for the Arts, Business and Leisure Services and the Riverdale Town Center.

I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE, the Centre for the Arts, Business, and Leisure Services, the City of Riverdale, its officers, servants, agents and employees (hereinafter referred to as RELEASEES) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage or injury, including death, that may be sustained by me or to any property belonging to me, while

participating in physical activity, or while on or upon the premises of Riverdale Centre for the

Arts, Business and Leisure Services and the Riverdale Town Center.

It is my expressed intent that this release and hold harmless agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representative, if I am

deceased, and shall be deemed as a RELEASE, WAIVER, DISCHARGE and CONVENTION

TO SUE the above named RELEASEES. I hereby further agree that this Waiver of Liability and

Hold Harmless Agreement shall be constructed in accordance with the laws of the State of

Georgia. I understand that in exchange for in consideration of my signature to this release, I am

being allowed to utilize the facilities at Riverdale Centre for the Arts, Business and Leisure

Services and the Riverdale Town Center.

In signing this release, I acknowledge and represent that I am unimpaired and of sound mind at the time of the execution of this release and that I HAVE READ THE FORGOING Waiver of

Liability and Hold Harmless Agreement, UNDERSTAND IT AND SIGN IT VOLUNTARILY as my own free act and deed; no oral representations, statements or inducements, apart from the

foregoing written agreements have been made; and
