Describe scope of palliative care and hospice and how these services can be used in a long-term care setting. Describe how to determine whether a long-term care resident is eligible for hospice and how to overcome barriers to referral.
Identify what the hospice benefit covers in terms of medication, and how to manage the relationship between the hospice, the facility, and the institutional pharmacy.
Understand how two highly-regulated industries (LTC and hospice) can provide mutually-beneficial partnerships and how hospice can support the staff at LTC facilities. Review specific relevant survey indicators (F-tags) as they relate to hospice services in the facility.
Describe billing issues that arise when providing hospice services to residents of the LTC facility.
Understand the specific roles of the hospice nurse, social worker, chaplain, volunteer and bereavement counselor and how those team members can support the LTC staff in caring for their resident at the facility.
