Developmental Pathways is very excited to be presenting our Fall 2016 Transition Forum & Fair!
The Transition Forum & Fair is an informational event intended to inform and empower individuals between the ages of 16-22, their families, caregivers, and educational providers.
Informational tables and presentations will take place to assist families, caregivers, and individuals in learning how to plan for, enroll, and benefit from the Supported Living Services (HCBS-SLS) Waiver while receiving Transition services through school!
*Light refreshments will be provided.*
The following information tables will be available from 9am-1pm:
- The Arc of Arapahoe and Douglas Counties
- The Arc of Aurora
- Community Outreach Department
- Division of Vocational Rehabilitation (DVR)
- Family Caregiver service options
- Family Support Council
- HCBS Waiver service options
Presentation Schedule (times are approximate):
9:00am: Doors Open
9:30-10:15am: Supported Decision Making (Presented by The Arc of Arapahoe and Douglas)
*Who makes decisions, Guardianship, and other alternatives
*Getting started, pre-enrollment prep, and what to expect during your enrolllment
11:15-11:45am: Waiver Services
*HCBS-SLS waiver--Specific services available thorough the waiver and how they can complement Transition services
*Learn about the ins and outs of Medicaid and Social Security including the tips and tricks to applying
*Spanish Translation will be available*
