Participants will explore evidence-based resources and strategies for organizing, developing, and monitoring a meaningful student-led transition program. This workshop will assist LEAs/districts in developing and delivering person-centered transition planning, from transition assessment resources to organizing school and community-based activities, to assist students in planning for college, career and civic life after high school.
