Description

We invite you for a day of education and celebration as we wrap up our CMS Transforming Clinical Practice Initiative journey through the Great Lakes Practice Transformation Network (PTN). During this time, we will discuss the impact our quality improvement work has had on the state and how quality improvement work is crucial in today's healthcare landscape. We will explore alternative payment models and how they align and support ongoing quality improvement in the Quality Payment Program and beyond. Additionally, we will examine how health information exchange is crucial in improving patient outcomes, and provide education and resources on how practices can treat opioid use disorder. Last, we will wrap up our full agenda with some reflections on Kentucky's contribution to the TCPI Aims and celebrate our successes! A detailed agenda is below for your review.
