Physicians and PAs: THMEP is accredited by the Arizona Medical Association to provide continuing medical education for physicians. THMEP designates this live activity for a maximum of 7.5 AMA PRA Category 1 Credit.
Physicians should claim only the credit commensurate with the extent of their participation in the activity.
NPs: This program is pending AANP approval for CE and Pharmacology.
RNs: Provider approved by the California Board of Registered Nursing, Provider No. 10161 Program Number2015-987 for 8.0 contact hours. The certificate must be retained by licensees for a period of four years after the course ends.

At the conclusion of the conference, you will receive an email with a link to complete the required online program evaluation. This is required for all CE and CME accedidation.
An additional follow-up survey will be emailed to you in December 2015. This survey is required for AMA CME credit.
Certificates will be emailed on December 15, 2015.
