Continuing Medical Education Credits:
Physicians, PAs, NPs: This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Arizona Medical Association (ArMA). Tucson Hospitals Medical Education Program (THMEP) is accredited by ArMA to provide continuing medical education for physicians. THMEP designates this live activity for a maximum of 7.5 AMA Pra™ category 1 credits. Physicians should claim only the credits that are commensurate with the extent of their participation in the activity.
RNs: Provider approved by the California Board of Registered Nursing, Provider No. 10161, Program Number 2014-921 for 8.6 contact hours. Certificate must be retained by licensee for a period of four years after the course ends.
ASRT: Activity approved by ASRT for 6.75 Category A Contact Hours.

Written requests for cancellation must be received by October 1, 2014. Registration fee (less a 10% handling fee) will be issued following the conference. Registrations for no-shows or cancellations received less than 2 weeks prior to the program date may be forfeited.
