Course Description

This course will provide the knowledge base and practical skills for use of casting materials to manage common impairments in pediatric and neurologic patient populations. Participants will learn to identify appropriate candidates for casting, problem solve which materials and techniques are appropriate for various clinical presentations, and evaluate progress. The course will emphasize the current evidence related to casting and foot/ankle intervention, the importance of differential diagnosis, and the use of the therapist’s manual intervention before and during casting to maximize therapeutic effects of casts. Attendees will learn to align casts for optimally for therapeutic gait and standing, and to problem-solve orthotic selection after casting is complete. Attendees will also learn to fabricate removable casts/splints. The course will include multiple opportunities to learn and apply hands-on practical skills which will can be carried over to the clinical setting after course completion. Audience: physical therapists and physical therapist assistants.

Course costs includes supply cost for multiple hands-on labs using a variety of casting materials. Participants will receive a detailed manual with guide to assessment, selection of casting materials, step by step instructions, and recommended competency assessment.

Registration is limited, early registration is recommended.

Prerequisites: None. This class is appropriate for clinicians without casting experience, or for those with casting experience who are looking to expand their repertoire of techniques and materials as well as expand their castings to include a manual therapy approach.

Course Objectives

Upon completion of the course, you will be able to:

  1. Select candidates for therapeutic casting using the ICF model, including rationale, assessment, considerations, precautions, and contraindications.
  2. Differentiate contributing factors influencing restricted range of motion at the ankle, including structural alignment, ground reaction forces, muscular pull, joint and soft tissue mobility, and directional susceptibility to movement.
  3. Apply a therapeutic cast with efficient body mechanics, selecting among casting techniques and materials based on patient presentation to direct forces to target structures and protect compromised tissues.
  4. Design casts to maximize therapeutic gait, using strategies to impact shank kinematics and kinetics with ground reaction force.
  5. Contrast orthotic options to carryover range and functional gains after serial casting.
  6. Contrast various materials used for and splinting, describing different properties and benefits for use for varied therapeutic goals.
  7. Apply multiple techniques for safe fabrication of splints and orthotics as appropriate based on patient presentation.
  8. Create a clamshell splint for hinge-joint stablization.
  9. Align a temporary AFO to maximize shank kinematics for therapeutic gait.
  10. Finish splints appropriate for function, ease of donning, and patient-centered design.
