OBJECTIVES

  • Explore how the Personal Directive helps adults with memory concerns, behavioral or mental issues, or those who cannot identify an Agent start their plan
  • Learn how to help adults start their plan using the Honoring Choices “Getting Started Tool Kit”
  • Review the 5 MA care planning documents to help adults update & add to their plan
  • Provide up-to-date information on MA MOLST & latest educational materials
  • Offer a free Alzheimer’s Association packet to engage adults with memory concerns
  • Review Palliative Care tools for consumers & care providers

ATTENDEES

  • Include: all care providers, community professionals, program managers, organizational/group volunteers
  • Will become Honoring Choices Health Care Planning Ambassador & receive Certificate
  • Will receive Ambassador Tool Kit & multi-lingual documents, slides decks & discussion
