Description

Thursday, January 18th 2018 – 6:00 pm- 8:00 pm

Come enjoy FREE dinner with your peers and learn about current best practices for treating tobacco dependence! All clinical care team members are welcome and encouraged to attend.

47% of members in Columbia Pacific CCO use tobacco - 47%! The highest rate in Oregon.

For 2018, CCO quality improvement targets a 1% reduction by the end of the year.

Join Pharmacist Melissa Brewster, Primary Care Innovation Specialist Maranda Varsik, and Behavioral Health Innovation Specialist Andrew Huff, as they present impactful best-practice guidelines and resources for effectively treating this challenging disorder. Tobacco dependence is one of our county's biggest challenges - together we can make a difference.

Attendance is limited, so please RSVP by January 5th.

Columbia Pacific CCO co-hosts this ongoing examination of disease states that:

• Highly impact patient quality of life

• Require complex management

• May lead to increased ED and hospital visits

Attendees gain valuable tools and insights, exploring different elements of patient medication risk, improving their ability to contribute as part of a multi-disciplinary team.
