The Child and Adolescent Needs and Strengths-NY (CANS-NY) is a collaboratively completed measure of child and family strengths and needs. This evidence-based, standardized assessment was developed to support decision-making, including level of care and intervention planning, to facilitate quality improvement initiatives, and to allow for the monitoring of outcomes. As a communication tool, it facilitates the linkage between the assessment process and the design of person-centered plans of care.

This presentation will provide an overview of the CANS-NY and of Transformational Collaborative Outcomes Management (TCOM). The principles and best practices in using the CANS-NY as a TCOM assessment strategy in NYS Health Homes for children will be reviewed, and strategies for using the tool to develop and monitor effective care plans will be presented.
