Description

An Advance Care Directive (ACD) planning workshop focuses on the importance of communicating your wishes should you be unable to speak for yourself. Our 2-part workshop will guide you through the thought process, your important conversations, and the planning needed to for you to complete your Advance Care Directive.

Dates:

Part 1: Wednesday, September 18th, 1:00 pm - 3:00 pm

Part 2: Wednesday, September 25th, 1:00 pm - 3:00 pm

Presented by Jewish Family and Children’s Services and Mission Hospice & Home Care.

You will learn about and have the opportunity to:
• Think about and advocate for what matters most to you.
• Talk about your wishes with your family and friends.
• Choose your health care agent.
• Write it down and complete your personalized ACD

Taking charge of your future care can bring peace of mind – to you and your loved ones.

Please plan to attend BOTH dates of this important workshop. Completion of this workshop can provide you with a notarized Advance Care Directive utilizing the California standard form.
