Registration will begin at 8am.

In an effort to improve care transitions between assisted living
facilities and hospitals, Society’s Assets is pleased to host this FREE
event to discuss issues that hospitals and emergency departments
face and how providers can assist in making these transitions of care
stress free for everyone.


It is clear that there are multiple factors along the care continuum that
impact both readmissions and Emergency room visits. Identifying key
drivers of these readmissions is the first step towards implementing
the appropriate interventions necessary for reducing readmissions
and ensuring that patients continue to live in the least restrictive
setting of their choice.


Our hope in bringing together these key stakeholders will be to work
together to improve the quality of care and the patient experience.


This panel discussion is recognized as a training need by the Department
of Quality Assurance and will serve as a “Kick off” event for the new
guide being produced by DQA for successful care transitions.
