Strive to Thrive: Self-Management Program For Parkinson's disease
Strive to Thrive is being offered in a partnership between Parkinson's Resources of Oregon (PRO) and The Parkinson Center of Oregon at OHSU (PCO). Based upon the Chronic Disease Self-Management Program (CDSMP) developed at Stanford University, Strive to Thrive will combine the proven self-efficacy theory of the CDSMP with the synergy of learning with and from people managing the same diagnosis of Parkinson's disease.
Participants will learn techniques necessary for managing chronic disease and can expect to leave the program with skills for self-monitoring, problem-solving, making informed decisions, and taking action to engage in healthy behaviors. Published research on CDSMP indicates measureable improvements in health and quality of life.
At this point, we are PRE-Registering individuals and couples for the 2017 classes. Times and locations through the year have yet to be determined. First choice will be offered to those who are pre-registered.
***This is a 7 week curriculum, class participants will be asked to commit to availability for the entire course as lessons build upon each other and participants rely upon each other for goal setting and accountability.***
Individuals diagnosed with PD as well as care partners are invited to attend
There will be a minimum of four classes offered in 2017 - all within the Greater Portland Area
The $25 materials cost of the program will cover the Living Well with a Chronic Disease book and the Relaxation CD that you will keep. Payment of the book will not be collected until you are enrolled in a specific class.
By attending this class, you will have the opportunity to participate in a research study conducted by Oregon Health & Science University. This will require you to fill out some information regarding the workshop and you may be contacting if you agree. However, this is not required to attend.
