Do you provide a service, activity or a group in Shrewsbury that supports health, well-being and independence?

Are you looking for local non-medical or clinical support for people you are working with?

If so, come and find out more about Shropshire’s social prescribing programme, how it is going to be delivered in Shrewsbury and how you can get involved.

Shropshire’s social prescribing model is led by the council in partnership with the voluntary and community sector and the Shropshire Clinical Commissioning Group. It is supported by primary care – many of our advisers are based in doctor’s surgeries – and our approach is to work locally with as many community groups and services as we can to offer people a wide range of support and things to do that will help them with the issues they are experiencing.

Many people are in situations that have a detrimental effect on their health and often people want to improve their situation, but need support and motivation to make a positive change.

It is estimated that around 20% of patients consult their GP for what is primarily a social problem. Our communities are full of groups, activities and services that can support people with these social problems. Social prescribing provides practitioners with a structured referral pathway into local, non-clinical community activities, services and support. It offers much more than signposting, as it includes support from an advisor to motivate and start to change behaviours, along with data recording and quality assurance of the services that are
referred to.

We are now going to deliver social prescribing from some of Shrewsbury’s medical practices and we need your help to make this a success. Please come along to one of the workshops to find out more.


Who should attend?

Anyone who is looking for non-medical/clinical support for the people they are working with. Voluntary and community groups, charities, social enterprises, commissioned services that provide activity for people in Shrewsbury that supports people to stay healthy and independent. People delivering community based services, e.g. libraries, creative groups and community centres. Practitioners and staff working in primary and secondary care – general practice teams, community health teams and pharmacies. People working in social care, mental health care, public health and health commissioning.
