About SNPT CAN:
Swansea Neath Port Talbot Community Advice Network (SNPT CAN) is a free direct referral system modelled on the Norfolk Community Advice Network, Tower Hamlets Community Advice Network and Oldham Community Advice Network – all in collaboration with The Children’s Society’s Coordinated Community Support Programme. In the other pilot areas, the system is now well embedded in the community, with a diverse range of organisations (schools, advice providers, social prescribers, council departments and housing associations) referring clients to one another through the system.
We are replicating the success of the referral system here, in Swansea and Neath Port Talbot, plus now have funding for Carmarthenshire. SNPTCAN allows organisations to make warm referrals to each other, which can help prevent duplication of work, assist the client quicker, and to promote partnership working.
