Beth yw Call4Concern?
Mae Call4Concern yn system sydd ar gael mewn rhai ysbytai yng Nghymru sy'n caniatáu i gleifion a'u perthnasau ffonio uwch aelod o staff yn uniongyrchol os ydynt yn poeni eu bod nhw neu eu claf yn mynd yn waelach. Ar hyn o bryd, mae'r uwch aelod hwn o staff naill ai'n rhan o'r tîm gofal critigol neu'r uwch reolwr ysbyty ar ddyletswydd.
Y cynllun yw cyflwyno'r gwasanaeth hwn i bob ardal ysbyty yng Nghymru dros y flwyddyn nesaf, ac mae angen eich help arnom i wneud yn siŵr bod system Call4Concern yn ystyrlon, yn ddefnyddiol, ac yn hygyrch i bob claf a theulu.

We're working with the Call4Concern team as they develop materials for this important new scheme.
What is Call4Concern?
Call4Concern is a system available in some Welsh hospitals that allows patients and their relatives to telephone a senior member of staff directly if they are worried that they or their one is becoming more unwell. Currently, this senior member of staff is either part of the critical care team or the senior hospital manager on duty.
The plan is to roll this service out to all hospital areas in Wales over the next year, and we need your help to make sure the Call4Concern system is meaningful, useful, and accessible to all patients and families.
