Self-care in Tameside and Glossop
In order to meet the challenges facing the health and care system, we need to fundamentally transform the way we support people to live healthier, make good choices about how they use health and care and to manage health conditions in partnership with services. A key part of the Care Together Programme is the development and implementation of a system wide self-care model.
The Care Together Programme will be running a number of exciting stakeholder co-design events. These events will inform and influence the implementation of the approach across Tameside and Glossop and will cover: system wide self-care, volunteering, asset based community approaches, social prescribing, Patient Activation, and much more.
This second stakeholder event will cover the following areas: Social prescribing, models of volunteering to support self-care, and importantly lots of great examples of great local work and opportunities to share practice.
