The Institute for High Quality Care (IHQC), in partnership with the California Improvement Network (CIN), invites you to a session where you will hear from practitioners in the field about how their organizations are currently screening and coordinating services to address their patients’ social needs.



Topics of Discussion:

Panel #1: Screening and Assessing for the Social Determinants of Health
A panel of care coordinators, directors, and executive leaders will share share the tools and strategies their organizations are using to screen for their patients' social needs, assess patient risk levels, and empower their care teams to use this information to inform follow-up services.


Panel #2: Care Coordination and Partnering with Community Organizations
A panel of practitioners, administrators, and leaders will share their lessons learned and challenges around coordinating access to non-medical services and developing and sustaining collaborative partnerships with community organizations.
