Population Health Concerns: Mental Illness
Population Health Management (PHM) seeks to improve the health outcomes of a group by monitoring and identifying individual patients within that group. In a given year, one of four persons will have a diagnosable mental health disorder. There is a clear, confirmed link between physical health and mental health. Patients with a serious mental illness, particularly older adults, have multiple co morbidity medical problems. This program will provide the nurse, regardless of practice setting, with the critical knowledge and skill set needed to manage any patient with a primary or secondary diagnosis of Mental Health.
