At the end of presentation, the participant will be able to

1.  Define the concept of care coordination
2.  Detail the importance of coordinated care
3.  Describe characteristics of care coordination models


Continuing Education:

Nurses: The Georgia Board of Nursing deems Southwest Georgia Area Health Education Center (SOWEGA-AHEC) as an approved provider for nursing continuing education (CE). This activity is approved for 1.0 contact hour towards the continuing education competency requirement for Georgia nursing licensure renewal. No partial credit offered. Activity #2018-04a.

Submission of registration information, attendance and completed evaluation required for nursing continuing education certificates.
