This two-day train-the-trainer course uses the standardized California POLST training curriculum, and includes a series of educational modules and interactive exercises designed to provide an understanding of the POLST Paradigm, as well as how to complete the form to reflect a patient’s wishes. This course is appropriate for physicians, physician assistants, nurses, LCSWs, LMFTs, nursing home administrators, chaplains and other healthcare providers working with the seriously ill and those nearing the end of life.

At the completion of this course, the active participant will be able to:

1)     Describe the purpose of the POLST Paradigm

2)     List the differences between an Advance Health Care Directive and the POLST form

3)     Differentiate between POLST and DNR forms

4)     Identify the target population for POLST completion

5)     Facilitate a POLST conversation with a patient/surrogate and document the patient’s treatment wishes on a POLST form

6)     Train others in the POLST Paradigm

Shirley Otis-Green is Clinical Director of Consulting Services for the Coalition for Compassionate Care of California.  Shirley is an award-winning licensed clinical social worker recognized by the Project on Death in America and the American Psychosocial Oncology Society. Her research focuses on quality of life, palliative care, and creating meaningful organizational change. Shirley is dedicated to enhancing excellence in the delivery of culturally congruent, person-centered and family-focused care. She is a Distinguished Social Work Practitioner in the National Academies of Practice and co-editor of the Oxford Textbook of Palliative Social Work.


Joanne Hatchett, MSN, RN, FNP, ACHPN
Family Nurse Practitioner
Woodland Clinic Medical Group

Joanne Hatchett has worked as a Family Nurse Practitioner in the Woodland Clinic Medical Group since 1995, caring for adult and geriatric patients in the clinic, acute care, and skilled nursing facilities. Since 2004, she has been the Palliative Care Coordinator for Woodland Memorial Hospital and developed the Bridge Program for Palliative and Supportive Care serving patients throughout Yolo County. She has coordinated the Yolo POLST Coalition since 2007. Joanne has been the lead developer of the California POLST Education Program. Joanne is a strong advocate for Advance Care Planning and patient and family education. She has collaborated and developed many educational tools for patients. Joanne graduated from San Francisco State University with a BSN in 1978. She received both her MSN as a Cardiopulmonary Clinical Nurse Specialist in 1983, and a Family Nurse Practitioner Post-Master’s certification in 1995 from the University of California, San Francisco.

Full attendance at the workshop is a prerequisite for receiving professional continuing education credit. Attendees must provide their professional license number when registering and complete an online survey within 30 days following the workshop to receive a CEU certificate. Certificates will be sent via email. PLEASE NOTE: 50 minutes minimum of uninterrupted time is required to qualify for one hour of CE credit. Continuing education excludes lunch and break periods.

Nursing – Provider approved by the California Board of Registered Nursing, Provider #CEP 15403, for 11.5 contact hours.
Social Work – Course meets the qualifications for 11.5 hours of continuing education credit for LMFTs, LCSWs, LPCCs and/or LEPs and required by the California Board of Behavioral Sciences. CCCC is approved by the California Association of Marriage and Family Therapists to sponsor continuing education for LCSWs and LMFTs, Provider #91987.
Chaplains - This program may be used for continuing education credit for chaplains certified with the Board of Chaplaincy Certification Inc.
