ABOUT THE TRAINING
This two-day train-the-trainer course uses the standardized California POLST training curriculum, and includes a series of educational modules and interactive exercises designed to provide an understanding of the POLST Paradigm, as well as how to complete the form to reflect a patient’s wishes.
At the completion of this course, the active participant will be able to:
1) Describe the purpose of the POLST Paradigm
2) List the differences between an Advance Health Care Directive and the POLST form
3) Differentiate between POLST and DNR forms
4) Identify the target population for POLST completion
5) Facilitate a POLST conversation with a patient/surrogate and document the patient’s treatment wishes on a POLST form
6) Train others in the POLST Paradigm

Full attendance at the workshop is a prerequisite for receiving professional continuing education credit. Attendees must provide their professional license number when registering and complete an online survey within 30 days following the workshop to receive a certificate of attendance.
Physicians and Physician Assistants - This Live activity, POLST: It Starts with a Conversation, Train-the Trainer, with a beginning date of 07/14/2016, has been reviewed and is acceptable for up to 10.5 Prescribed credit(s) by the American Academy of Family Physicians. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Nursing – Provider approved by the California Board of Registered Nursing, Provider #CEP 15403, for 11.5 contact hours.
Social Work – Provider approved by the California Association of Marriage and Family Therapists, Provider #91987, for 11.5 contact hours.
Nursing Home Administrators - Approved by the Nursing Home Administration program for 12 hours of continuing education credit – NHAP# CEP 1735.
Chaplains - This program may be used for continuing education credit for chaplains certified with the Board of Chaplaincy Certification Inc.

A $25 non-refundable processing fee will be retained for each cancellation. No refunds will be made after this date. No shows are subject to the full registration fee. Substitutions are encouraged.
