Important Information
This clinic is for first or second doses of the Pfizer vaccine. This vaccine has been authorized for those age 12 and older.
What to Bring:
• Please bring a government-issued photo ID (e.g., driver’s license) for identity verification. To expedite you appointment, please provide a photocopy of this ID.
• Please also bring your health insurance card. You will be asked to provide the name, date of birth, and Social Security # for the policy holder of your health insurance plan. If you are not the policy holder, please bring this information with you at the time of vaccination. To expedite your appointment, please provide a photocopy of your health insurance card.

• If this is your second dose, be sure to bring the vaccination card you were issued when you received your first shot.
• To further expedite your appointment, please print and complete the Vaccination Consent Form. Copies of this form will be available if you are unable to print it in advance.
Arrival Details:
• Please enter through the Medical Plaza Entrance. You will follow the hallway around to the right and walk through the atrium between the Medical Plaza and the Hospital. Please stop at the screening desk to have your temperature taken and to answer standardized screening questions.
• Follow the green signs to the Surgery Waiting Room on the first floor of the Hospital for check-in. A staff member will review your paperwork and give you further instructions from there.
• Please arrive as close to your scheduled appointment time as possible. We are unable to accommodate early arrivals. If you arrive more than 15 minutes before your scheduled time, you will be asked to kindly wait in your car. Excessively early or late arrivals will slow down the process for everyone.
• Please schedule at least 60 minutes for your vaccination. Although we have listed arrival times in 15-minute intervals, your actual wait time may vary. You will also need to stay for 15 minutes after receiving your shot for observation. Thank you for your understanding and patience.
• While waiting for your vaccine, please remember to adhere to COVID-19 safe practices, including wearing a mask and maintaining a distance of 6 feet from those outside your immediate household.
• If you need to cancel, please do so through Eventbrite as early as possible in order to give others the opportunity to register.
Vaccine Details:
• COVID-19 vaccinations at NNRH will be administered in accordance with current guidelines from the State of Nevada to individuals who are eligible for the vaccine.
• Possible side effects include fever, body aches, fatigue, and headache. For this reason, it is recommended that those receiving this vaccine attempt to schedule prior to being away from their job for at least the following 24 hours.
Vaccine Warnings & Precautions:
Talk to your doctor before you are given the vaccine if you have...
• a known history of a severe allergic reaction (e.g., anaphylaxis) to any component of the COVID-19 vaccine.
• are pregnant, or breast-feeding, think you may be pregnant or are planning to have a baby. As a precaution, you should avoid becoming pregnant for at least 2-3 months after the vaccine.
• a severe illness with high fever. However, a mild fever or upper airway infection, like a cold, are not reasons to delay vaccination.
• a weakened immune system, such as HIV infection, or are on a medicine that affects your immune system. Immunocompromised persons, including individuals receiving immunosuppressant therapy, may have a diminished immune response to the vaccine.
• a bleeding problem, bruise easily or use a medicine to inhibit clotting.

What if I’m uninsured? We will still provide the vaccine at no charge to you.
I thought the vaccine was free. Why is my insurance being billed? The vaccine is provided to all patients at no cost to them. However, we will bill your insurance carrier for the cost to administer the vaccine, as the government has put measures in place that allow healthcare providers to be reimbursed for their efforts to vaccinate the American people. You should not receive a bill for this – it is purely administrative with your carrier.
