Pediatric Population Health Forum - Saturday January 21, 2017
Mary Bridge Children's Hospital & Health Center, WCAAP, and the Pediatric Transforming Clinical Practice Initiative are excited to bring you a statewide event on health care transformation in pediatrics and improving the social determinants of health. We will dive in to a full day of learning and discussion on accountable care, care management and psycho-social determinants of health including:

- ACE curriculum in primary care: what do to with a positive score;
- Behavioral health integration;
- Community health workers and asthma, and significance of social and environmental control.
