Mary Bridge Children's Hospital & Health Center, WCAAP, and the Pediatric Transforming Clinical Practice Initiative are excited to bring you a statewide event on health care transformation in pediatrics and improving the social determinants of health. We will dive in to a full day of learning and discussion on accountable care, care management and psycho-social determinants of health including:

  • How to care manage children in a population health frame;
  • Accountable communities of health and alignment with health care transformation;
  • Patient-Centered Medical Home experience;
  • ACE curriculum in primary care: what do to with a positive score;
  • Behavioral health integration;
  • Community health workers and asthma, and significance of social and environmental control.

Learn what this will look like when integrated into your practice, and take home practical strategies.
