CSH and Los Angeles County Supervisorial District 2 invite you to two half-day convenings about the opportunities available as a result of the most recent reforms to California Health systems. These reforms include the creation of a Drug Medi-Cal Organized Delivery System and the Whole Person Care pilot offered through the State’s 1115 Medicaid Waiver, and a new program California is developing to take advantage of the “health home option,” to provide housing-based services as a new Medi-Cal benefit. These opportunities promote integration of care across systems of care.

Part I
Tuesday, April 25, 2017

This convening educated providers on how the County plans on taking advantage of all three opportunities, and how providers can tap into funding available to be better able to serve existing and new clients.

Part II
Wednesday, June 14, 2017, from 9:00 AM – 2:00 PM PDT

This convening will focus on potential County actions to coordinate these opportunities, with the goal of improving health outcomes among three vulnerable populations: Los Angeles County residents who are homeless, who are exiting jails, or who are frequent hospital users. Specific topics include integration of care and workforce development.


BENEFITS
  • • Participate in discussions on ensuring health opportunities promote integrated care for our most vulnerable residents;
  • • Help develop recommendations for creating the work force the County will need to use newly-available funding; and
  • • Engage in dialogue on how SD 2 providers can access funding opportunities.
  • This session will lead to a series of recommendations for County action. Make sure you weigh in!

All are welcome—you don’t need to be working in the health or housing world to benefit from this workshop.

The Intern ad Residents Building is located towards the back of the MLK Medical Campus, behind the newly remodeled Outpatient Center.
