BY REGISTERING FOR OPEN GYM AT MY PLAYHOUSE YOU AGREE TO THE FOLLOWING:

I hereby give my consent to my child's participation in the activities of MY Playhouse and Cathay Washington Future Centerand hereby absolve, release, and hold harmlessMY Playhouse and Cathay Washington Future Center and all of its officers, directors, shareholders, agents, representatives, attorneys, employees, owners, successors, and other affiliates from any and all liability for any injuries or damages that my child may suffer in connection with the activities sponsored by MY Playhouse and Cathay Washington Future Center in which my child may participate.

I recognize that participation in the activities including, but not limited to Open Gym Play Times, Classes and Special Eventsat MY Playhouse and Cathay Washington Future Center, even when well managed, poses a risk to my child and me, and I agree to assume such risk for myself and on behalf of my child. I recognize that MY Playhouse and Cathay Washington Future Center do not provide supervision during My Playhouse Open Gym Play times and as a parent/caregiver I am solely responsible for the care and well being of my child.

I give permission to My Playhouse to take photos of me and my child during open gym and allow My Playhouse to use these photos on their social media and also in their advertising.
