Neighbourhood health and care for people with multiple long-term conditions
Overview
Brief Overview:
People with multiple long-term conditions (MLTCs) are identified by the NHS and government as a central part of the priority cohort to build neighbourhood health services for. This webinar will be based on the Richmond Group of Charities’ upcoming report on MLTCs and neighbourhood health, exploring the role of clinicians and system leaders in improving experiences and outcomes for people living with multiple long-term conditions. Attendees will hear about good practice (developed via engagement with patients, NHS, local authorities and the VCSE), how areas like theirs have tackled challenges to moving to more integrated neighbourhood ways of working, and recommendations from the report on how they can make this happen.
Learning Outcomes:
- Attendees will gain an understanding that people living with multiple health needs have the most to gain from effective neighbourhood health approaches.
- Attendees will leave the session equipped with practical ideas/tools for how they can improve outcomes and experiences for patients living with multiple long-term conditions.
