Hosted by Nationwide Children’s Hospital and Partners For Kids, this CE lunch and learn will provide insight into what care coordination is and how all disciplines within the care team can work together to improve patient outcomes.

The Learner will obtain an understanding of the role and scope of care coordination, how to manage care coordination responsibilities, and how to meet daily patient demands. The learner will also understand how to practice inter-disciplinary collaboration with care coordinators to assist patients and families with transitioning towards self-management and family-centered care.

1 contact hour is available. You must attend 80% of the Program AND submit a program evaluation in order to receive CE Credit.

Nationwide Children's Hospital is approved as a provider of continuing nursing education by Ohio Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. (OBN-001-91).

The faculty and planners of this program have no conflicts of interest.

Lunch will be provided to the first 50 in attendance. Please bring your own beverage.
