Event description

Description

This event will convene 125 leaders and learners from Rhode Island, Massachusetts and across the country for a dialogue on assuring that the social, economic and environmental conditions impacting people's health - social determinants of health (SDOH) - are key drivers for equity as healthcare delivery shifts from volume-based to value-based care. We will feature:

- An energetic keynote address from Dr. Nicole Alexander-Scott, who is leading innovation in health equity at the Rhode Island Department of health and also is Presidnet-Elect of the Association of State and Territorial Health Officials (ASTHO); and

- A "deep-dive" panel on a comprehensive framework for high-quality social health integration in accountable care. This cross-sector dialogue will help shift the conversation about social determianants of health from "why" to "how" at a time of unparalleled opportunity and urgency. Speakers will address:

  • Innovation in risk adjustment methodologies to account for SDOH;

  • Strengths-based screening of individuals and families for SDOH;

  • Evidence-based SDOH intervention strategies that transcend the simple screen-and-refer paradigm; and

  • Workforce preparation for high-stakes SDOH responsibility and accountability
