Event description
Participants will be provided basic concepts to help build or partner with, operationalize, and sustain their own MIH/CP systems.

About this Event

This conference is designed to provide participants (local government representatives, EMS agencies, EMS Agency Operational Medical Directors, hospital administrators, and community health partner organizations) some basic concepts to help build or partner with, operationalize, and sustain their own mobile integrated healthcare systems.

Changes in medicine requires a collaborative approach to achieve the broad goals of the Institute for Healthcare Improvement's "Triple Aim" of improving the patient experience of care (including quality and satisfaction), improving the health of populations, and reducing the per capita cost of healthcare. Many groups have a role in achieving improved population health such as health systems, primary care physicians, home health agencies, and EMS agencies.

Mobile Integrated Healthcare (also known as Community Paramedicine) allows EMS providers to function outside their traditional emergency response and transport roles to help facilitate more appropriate use of emergency care and community healthcare resources while preserving prehospital access to primary care for medically underserved populations. This system helps reduce the misuse of EMS and hospital emergency departments while improving the overall patient care experience and reducing overall costs. Community paramedicine generally focuses on:

  • Providing and connecting patients to appropriate, cost effective and targeted primary care services
  • Completing post hospital follow-up care
  • Providing education and health promotion programs
  • Not duplicating available services in the community
